Opt-out forms for March 2022 through February 2023 must be submitted in person at York Township Hall, 6609 Norwalk Rd., Medina, Ohio 44256, on Monday, January 3^rd, 2022 from 4pm to 7pm.

Forms will be time-stamped when dropped off and residents will be placed on the opt-out list accordingly.  Forms cannot be accepted or processed before Monday January 3^rd, 2021 due to the holiday.

There will be no mail-in, early drop off or computer applications accepted this year.
