Opt-out forms for March 2022 through February 2023 must be submitted in person at York Township Hall, 6609 Norwalk Rd., Medina, Ohio 44256, on Monday, January 3rd, 2022 from 4pm to 7pm. Forms will be time-stamped when dropped off and residents will be placed on the opt-out list accordingly.  Forms cannot be accepted or processed
